Highly-rated Willow Brook Christian Village is seeking a full-time Licensed Social Worker for our smaller SNF and Assisted Living care centers. The social worker will plan, develop, organize, implement, and direct all facets of the social services programming. Candidates must have experience and a genuine interest in working with older adults in a nursing care setting of which 35-40% of residents are skilled care residents. A pleasing personality and ability to relate well with residents, families and co-workers is essential. Schedule is 40 hours per week, primarily weekdays from 9 a.m. to 5 p.m. and includes some needed availability for occasional evenings and weekends. Vital job responsibilities include identifying and addressing the psycho/social needs of residents, completing comprehensive resident assessments, assisting residents and their families in adjustment to placement and rehab/illness/aging process, provide ongoing support to residents/families, providing Medicare/Medicaid information, assisting with advanced directives, insurance pre-certifications and submissions, ordering equipment, coordinating care and discharge plans and community re-entry, and engaging the support of outside sources as needed.
Education requirement includes a Bachelor’s degree in social work and a valid license as a Licensed Social Worker (LSW) or a Licensed Independent Social Worker (LISW). Candidates must possess good communication skills, as well as a courteous, patient, and understanding demeanor. Must maintain resident confidentiality and be highly dependable. Computer literacy and a nursing care background are essential.
